Location: 4th & Delaware (Lincoln Room, lower level), Leavenworth - Park in the parking lot on the south side of the building (corner of 4th & Cherokee) and enter through the south entrance to the building.  Proceed down the stairs to the Lincoln Room, or through the interior set of glass doors to the elevator to the basement. 

Midwest Homeplace

Location: 419 and 423 Limit, Leavenworth

Description: Adult group home

Visit Notes: day room visit, single story, residents are ambulatory

Meet & Greet: Meet at the front driveway between the two buildings 10-15 minutes prior to the visit.

Lansing Elementary

Location: 450 W Mary St, Lansing

Health Certificate Requirement: On your first first bring a copy of your dog's health certificate which will be retained by the school nurse.

Description: Early childhood special education, developmentally delayed children

Visit Notes: classroom, single story, all linoleum floors.

Meet & Greet: Enter thru the middle building to the office. Report in there and they will help you make you photo badge on their computerized visitor's equipment.  When you leave you also are to check out on the same computer.

Directions to the School : Turn right (west) off of Hwy 7 south at the stop light of East and West Mary in Lansing.  Travel past the apartments on the left , go straight and follow the road down into the valley.  The school will be on the right.  Take the first street on the right down into the parking lot. 

Ross Hall

Location: 4100 South 4th Street (on the St. Mary College Campus), Leavenworth

Description: Skilled care center for retired nuns, convalescent infirmary for practicing nuns

Visit Notes: single story, tile floors, residents are ambulatory and non-ambulatory

Additional Notes: It is easiest to access this facility from the Hughes Road entrance. A small white "St. Mary College" sign denotes the entrance.

Meet & Greet: Meet under the car port/canopy on the west side of the facility 10-15 minutes prior to the visit.

St. John Hopsital

Location: 3500 South 4th Street, Leavenworth

Description: 1 South--skilled nursing unit / 2nd floor--geriatric psychiatric care

Visit Notes: brief day room visit, room-to-room visits, tile floors, carpeted floors, elevator, must phone at 2nd floor entrance for admittance

Additional Notes: Both units are visited on the same day. Human members of the team must have an annual negative TB test on file with the infection control nurse and the animal member of the team must have a current health certificate on file with the infection control nurse. This visit is quick to fill and is limited to four (4) teams per visit.

Meet & Greet: Meet under the canopy on the main entrance (east side) 10-15 minutes prior to the visit.

Veteran's Affairs Building 122

Location: 4101 South 4th Street, Leavenworth

Description: 40 bed nursing home care unit

Visit Notes: escorted visit, elevator, room-to-room, tile floors, residents are non-ambulatory

Additional Notes: Enter VA complex at the main entrance off 4th Street. Go through the main gate & immediately at the "T" intersection, turn right onto South Drive. Continue past church on left side of road. At pond, turn left onto Lakeside Drive. At the "T" intersection turn left onto Franklin. Building 122 is a three story brick building immediately on your right.

Meet & Greet: Meet by the pond 10-15 minutes prior to the visit.

Twin Oaks

Location: 657 Eisenhower, Lansing

Description: assisted living

Visit Notes: day room visit, carpeted floors, single story, residents are ambulatory

Additional Notes: There are two buildings that comprise Twin Oaks--a three story retirement community and a single story assisted living facility. Our visits take place at the single story assisted living building. Enter off Eisenhower & turn right.

Meet & Greet: Meet at the main entrance (north side) of the single story building 10-15 minutes prior to the visit.

Veteran's Affairs Domiciliary (DOM)

Location: 4101 South 4th Street, Leavenworth

Health Certificate Requirement: New volunteers are required to provide the volunteer coordinator a copy of your health certificate.

Description: Assisted Living

Visit Notes: Residents come to central atrium, tile floors, residents are ambulatory

Additional Notes: Enter VA complex at the main entrance off 4th Street. Go through the main gate & immediately at the "T" intersection, turn left onto Rowland. Proceed about 1/4 mile and turn left at domiciliary sign. The DOM is building 160 & is two stories with a green roof.

Meet & Greet: Meet at the fountain on the south side of the building 10-15 minutes prior to the visit.
