St. John Hopsital

Location: 3500 South 4th Street, Leavenworth

Description: 1 South--skilled nursing unit / 2nd floor--geriatric psychiatric care

Visit Notes: brief day room visit, room-to-room visits, tile floors, carpeted floors, elevator, must phone at 2nd floor entrance for admittance

Additional Notes: Both units are visited on the same day. Human members of the team must have an annual negative TB test on file with the infection control nurse and the animal member of the team must have a current health certificate on file with the infection control nurse.

Meet & Greet: Meet under the canopy on the main entrance (east side) 10-15 minutes prior to the visit.
